The design with the fridge sitting on top of the drawers is also more practical when you don't have the fridge on board.
The Black Widow system I have has a flush mounted slide which gives you a totally flat surface for storing and carrying other goods.
If you have the fridge down low and two drawers stacked on top of each other, you have no flat space in the rear.
I have no problem with the 40 litre Engel sitting in the back of the Jack. My wife gets by, as the Jackaroo has a built-in step at the rear, so she just steps up to reach into the fridge when it's in the vehicle.
I tend to remove the fridge when we
camp so any perceived problem is eliminated altogether.
It gets down to what "compromise" suits you best.
My suggestion is to work out what method suits your individual use and also taking into consideration how big the fridge is and go from there.
Most manufacturers have a range of different configurations to choose from.
