Misfiring 2006 td courier

Had an intermitant misfire on a 100klm run yesterday.
Local servo had a look and pulled the fuel filter, heaps off rubbish in filter, put new one on and still had misfire.
The mech then removed a small gauze filter from after the filter and hey presto it had a little ball off what looked to be fibres, so that when you put the foot down it blocked the fuel but at lower revs did not affect the motor at all.
This is after the filter so how did it get there?
Filter has been changed as per the book.
Most of my diesel has been from safeway but that will now change.

Lint filter blockage is very common bushy,it actually traps the loose particles from the filter element hence why it is called a lint filter and more prolific filter replacement will result in more lint to catch.
