rattles in 1st,2nd and 3rd gear nothing happens in 4th but rattle gets louder in 5th , seems to stop rattling when accellerator is released with truck still moving

I assume you mean the gearbox is making the noise. It sounds like the cluster gear bearings are worn. When in 4th the cluster gear has no load on it. The only fix is a gearbox rebuild or replacement.

have you checked to see if there is any oil in the g/box you havent said what model nissan how many gears / age or how many km's its done
so often is the case on here
a lot of the time its hard to make comments about the given problem when the posters dont give much detail
not having a go at you just suggesting if posters with problems gave more basic imfo up front you are more likely to get a more satisfactory answer
