Hilux 2.8 diesel problem - continued

Okay, worked fantastic for a week and has come back again.

Problem appears in any gear with full or close to full throttle under load mostly.

At about 3200 revs and above it starts to missfire? or runs rough, loses power and billows a blueish smoke, quite significant amounts!

bring the revs down and it runs fine. Then the next startup it takes a few more revs to start?

Does this sound like a fuel starvation problem/air leak.

Does the injector pump have a vaccuum advance?

Could it be an injector playing up?

Does anyone know a good\reputable diesel mechanic in Perth, northern suburbs preferably that you can recommend.
